In the 26 days that Mr. F was at the community. He participated in rehab, drastically improving his balance and walking distance. Mr. F could only balance/stand for under 30 seconds, then stand for 5 minutes or more. When he first came, he could only walk 10 feet, then over 300 feet. Since he was doing very well with therapy, he was ready to go home.

She could also ambulate over 500 feet independently and negotiate over 13 steps. She became very close with her therapist and social worker. She stated, “St. James is the only place I will go for rehab.

The art therapy was terrific, and so were the activities she did throughout her stay. She thanked everyone who helped her and kept her entertained.

Ms. G had a fall and, thankfully, was found by her neighbor. She was then airlifted to Stony Brook University Hospital (SBUH). EMS intubated her, and as soon as she was at the hospital, they called a trauma team.

As he was getting ready to discharge from our facility, he stated, “I am going to my new home, but St. James is where I will always come back to. It is like a vacation home with all my family.”

As the concierge assisted him with his discharge, Mr. Pawluk stated, “I can’t wait to come back here and mentor other patients who are just like me and let them know it will all work out. I had so much support from the therapy team, the comprehensive team, the concierge, and the administrator. I can’t thank all of them enough.”

As the concierge was helping him get ready to discharge, Mr. V stated, “This is a wonderful place to get better. Not just physically but mentally as well. I had so much support and made so many friends. I will always come back.”

Mr. Miller stated, “I love working with the staff here. They make me feel 100 times better by the time I am ready to return home. I hope if I ever need to come back here that they always have a bed for me.”

Ms. Woodard spent 389 days at St. James, working hard every day to meet her goals. Reflecting on her time with St. James, she stated, “I’ve been to other facilities, and the difference with St. James is all the employees work with their heart. St. James has heart.”

Reflecting on her time with St. James, she stated, “I wouldn’t be able to get back up each time without the amazing staff. The time they have spent with me and explaining the process of each step is just so incredible.”

Theresa has participated in the “on-unit” bingo games and likes to watch TV, relax and recover. Although Theresa does not have a discharge date as of now, the team all believes in the strides she has made and looks forward to helping her along her way back home.

She made friends with several people on her unit and shared some impactful and touching words with everyone as she was being discharged.

Reflecting on his time with St. James, he shared, “Ed and Nicole were fantastic! The time they spent with me was incredible”, adding, “They were unbelievable!”

Marie says her family came to visit her shortly after and she stood up in her wheelchair to greet them, which they couldn’t believe! She is more than pleased with the physical therapy and occupational therapy she received and would rate them 100% for her 100 days of intervention.

Once Mrs. Alvarado’s discharge date was set, she was eager to get home. She went around telling everyone about her upcoming departure and there were cheers and sadness because we truly were so happy to have her here.

The goal with therapy was to get her to walk again, eat on her own, and to be able to do everything that she was doing before she had the operation.

Mr. Mays was in Stony Brook Hospital for one month and then stayed an extra six days in the hospital before getting discharged to St. James. He has been here multiple times in the past 10 years, so he knew he wanted to come back here for rehab because he was familiar with the community and really liked it.

We at St. James are so proud of Ms. Dimaio for all of her accomplishments she made here and wish nothing but the best for her in rest of 2018!

The Case Management team at St. James also arranged for Thomas to receive continued clinical support while in his own home. Team St. James is thrilled for Thomas and the incredible strides he achieved while under our care.

On January 30, 2018, Mrs. Orozco was discharged home with an eagerness to see her children, her grandchildren and spend quality time with her husband in their own home. Upon discharge, her ambulation increased from 100 feet to 200 feet and she was able to transfer independently.

On December 29, 2017, Mrs. Chmela was discharged home to celebrate the New Year with an eager return to her family. Upon Discharge, her ambulation increased from 100 feet to 200 feet and was able to transfer independently, without any supervision.

On November 29, 2017, Mrs. Alvino was discharged home with an eager excitement to celebrate the holidays with her family, and friends.

The social services team at St. James center also arranged for Linda to receive continued clinical support through an out-patient services as she transitions back home.

Upon discharge, his ambulation increased from 100 feet to 200 feet and he was able to transfer independently achieving his goal.

She cannot wait to utilize all the great qualities and tools she has gathered from St. James Rehabilitation and Healthcare at her own home. Her goal is to be able to motivate herself to continue all her wonderful therapy.

Mr. Lieb’s discharge date is soon nearing, and even though he loved it here with all of us at St. James, leaving will be a bittersweet moment.

Upon discharge, Mrs. Spengler shared, “ I want to give well wishes for the staff, nurses, and therapist here at St. James. They were all very good, and always put the patients first.”

Roy has built a connection through friendship with many of our staff here at St. James. He shared, “this is my second home. Even my daughter works here. It feels like a welcoming atmosphere.”

Elliot was quickly welcomed into our community by each department at the time of his admission. Elliot was very impressed on how well each department worked together.

Both he and his family were educated on the importance of proper medication administration and safety. Upon discharge, notable improvements were made; he only required minimal assistance to perform ADLs and was ambulating between 100 to 150 feet.

Upon his discharge, he was ambulating 125 with CGA, he was able to transfer and perform ADLs independently.

After 2 weeks here, the resident began to show much improvement. Resident was doing her exercises great with supervision, being able to walk 250ft with supervision, standing with assistance 1-3 minutes, and going up to 8 steps with support.

Resident was admitted to St James Rehab with admitting diagnosis of congestive heart failure, fluid overflow and lower extremity cellulites.

Through continuous support from the entire staff at the facility he make great strides and was able to return to his home in the community.
