St. James Rehabilitation & Healthcare Center (September 2015)

Patient was originally admitted to St. James Rehabilitation Center after a lengthy stay at SBUH due to gangrene of the leg. In addition, he had numerous co-morbities including Congestive Heart Failure, COPD, hypertension, and Diabetes. When he originally arrived he was lethargic and unable to participate in therapy for the entire time. He experienced a tremendous amount of pain and struggled just to roll in the bed. The wound care physician closely followed the patient due to the gangrene that he was suffering from, she believed that in order to succeed in therapy that the leg would have to be amputated. A month into his stay, he was lethargic, hypotensive, and had a significant change in mental status. Even with IV hydration running he remained hypotensive. He was transferred to SBUH and remained in the hospital for a month and a half. He underwent surgery and had an above the knee amputation. When he returned at the end of April, he did show signs of depression and need help adjusting. With the help of Creative Arts, psychotherapy, and the support from the staff, he accepted and embraced his new body image. In addition, he worked extensively with both physical and occupational therapy. He attended physical therapy six times a week for difficulty with walking, balance activities, gait training, and safety education. He also attended occupational therapy five times a week for therapeutic exercise, therapeutic activity, ADL retraining, neuro-muscular re-education, and wheelchair management. Upon his discharge he was able to manage his prosthetic, ambulate with assistance, and provide self care Through continuous support from the entire staff at the facility he make great strides and was able to return to his home in the community.
