Patient was admitted to St. James Rehabilitation Center after a nine day stay at SBUH. There he had a CABG x 4. Upon his admission, he had weakness, difficulty ambulating, and required some assistance when performing self care. He attended physical therapy six days a week for difficulty with ambulation. He also attended occupational therapy six times a week for bed mobility, transfer, toileting, activities of daily living, stand balance tolerance, safety, and energy conservation. During his stay he was closely monitored by the in house cardiology group of North Suffolk Cardiology. In addition to Physical and Occupational Therapies he also attended Cardiopulmonary rehab. There his EKG and vital signs were monitored by respiratory therapist to no changes in his EKG while ambulating and performing light exercises. He was also followed by his primary care physician weekly, who closely monitored his labs and medications and adjusted them accordingly. Prior to returning to the community he received education on medication administration and potential side effects. Upon his discharge to the community, great strides had been made in his ability to perform self care, ambulate, transfer, and safety.
