St. James Rehabilitation & Healthcare Center (December 2015)
Patient was admitted to St. James after a hospitalization at SBUH for a mitral valve replacement and CABG x 3. He had a history of ESRD and received dialysis three times a week while in the facility. He also had bilateral pleurex catheters that were drained by the nurses three times a week, to minimize the fluid in his lungs. Upon admission, patient was noted to have weakness, an unsteady gait, and a decline in overall ADL functionality. He was unable to ambulate. He attended PT and OT five times a week to improve difficulty with ambulation, balance and gait. He also attended cardiopulmonary with the respiratory therapist weekly to monitor blood pressure and heart rate while ambulating and exercising. Through compliance of medications and treatments, attending therapies, and following up with his physician, patient made great improvements. Upon his discharge, he was ambulating 125 with CGA, he was able to transfer and perform ADLs independently.
