St. James Rehabilitation & Healthcare Center (February 2016)
Patient was admitted to St. James Rehabilitation Center after an admission to SBUH due to a fall that resulted in a fracture of the right hip and an ORIF. Upon her admission, she had a noted decline in her ability to perform her activities of daily living, as well as weakness and difficulty transferring. She attended physical therapy six days a week for therapeutic exercise, gait training, and therapeutic activities. She also attended occupational therapy six times a week for therapeutic activities, therapeutic exercise, ADL re-training, and wheelchair management. During her stay she was followed weekly by her primary care physician that followed her case in the facility to ensure that she remain medically stable.
In addition, she was followed by in house consultants including: the wound care physician to monitor healing of any impairments to her skin and the in house psychology group to ensure adjustment to the facility. She became an active participant in a variety of activities provided by therapeutic recreation including: musical entertainment, morning social, special events, sing-a-longs, and pet visits. Through out her stay she remained medically stable, her surgical incision healed, and her ability to perform ADLs improved. Upon discharge, she reached her goals set by physical therapy, occupational therapy, and nursing.
