Patient was admitted to St. James Rehabilitation Center after a lengthy stay at SBUH for Acute CHF that resulted the need for him to be on a continuous cardiac drip. Upon his admission, he was noted to be de-conditioned. He had notable weakness and needed assistance with his ADLs. He attended physical therapy six days a week for therapeutic exercise, gait training, and therapeutic activities. He attended occupational therapy six times a week for therapeutic exercise, therapeutic activities, ADL retraining, neuro reeducation, wheelchair management, and caregiver training.

In addition, to physical and occupational therapy, he attended cardiopulmonary rehab where his vital signs and EKG was monitored as he performed various exercise. During his stay he was followed weekly by his primary care physician to ensure that he remained medically stable. He was also seen frequently by the in house cardiologist of the North Suffolk Cardiology Group, to monitor his cardiac stability. The continuous cardiac drip and PICC line was managed by the nursing staff on the sub acute floor. He was educated through out his stay on being compliant with medications and treatments. Both he and his family were educated on the importance of proper medication administration and safety. Upon discharge great improvements had been made in his ability to perform self care, ambulate, transfer, and in safety awareness.
