Patient was admitted to St. James Rehabilitation Center after his admission to St. Catherine’s due to dehydration, acute renal failure, and a CABG x 4. Upon his admission, he had noted weakness, difficulty transferring, and difficulty performing ADLS. He attended physical therapy for gait training, difficulty with ambulation, and balance. He attended occupational therapy for bed mobility, ADL’s, self-care, transfers/ toileting, and stand tolerance/balance. In addition to physical and occupational therapy, he worked with the respiratory therapist for cardiopulmonary rehab. He attended cardiopulmonary rehab weekly and made great strides in PT and OT; his ability to perform ADLs, ambulate, and balance improve greatly. During his stay he was monitored by his primary care physician as well as the cardiologist from North Suffolk Cardiology. He was compliant with medications and treatments through out his stay, and received education on medication administration to ensure safety when at home. Upon discharge great improvements had been made in his ability to perform self care, ambulate, and transfer. He frequently returns to the facility to visit the staff and patients he befriended.
